SOCIAL PRESCRIBING LINK WORKER
37 hours per week – fixed term to January 2020 (ideal secondment opportunity)
Urdu and Punjabi speaker essential
Salary £18,672 to £20,541 (SCP scale 17 – 21), dependent on experience
5% pension contribution
22 days holiday + bank holidays and 3 concessionary days
VAST is currently looking to recruit a Social Prescribing Link Worker to improve the health and wellbeing of people in the local community, in particular the BAME (Black, Asian and minority ethnic) population, through 1-2-1 engagement with service users, and to provide access to, and information about, Voluntary, Community and Social Enterprise sector (VCSE) and the services they provide to health professionals
In addition, the role will help to identify gaps in service provision to help inform commissioning intentions and stimulate the VCSE market around potential opportunities to develop services to fill gaps.
