The VCS Hub is a free service offering social prescribing support covering North Staffordshire. It helps individuals and groups to access support within the voluntary and community sector.
Anyone with social, emotional or practical needs can benefit. It can be particularly useful for vulnerable or isolated groups, and those accessing primary or secondary health care with additional support needs.

How does it work?
Contact is mainly by telephone, with some face to face visits. If you’re over 18, you can self-refer. Alternatively, individuals can be referred by health and social care professionals, or by voluntary sector workers.
The Voluntary and Community Sector Hub (VCS Hub) will take the initial referral. They’ll explain what is available and help you start getting the support you need.

All referrals are treated in the strictest confidence. To help us improve the support we offer, details may be shared with the individual’s permission. Follow-up calls are also included in this service. This helps to measure outcomes and also provides feedback for support partners.
