VAST provides a range of strategic support to health and social care services and works closely with local authorities, CCG’s and other providers to support the NHS 5 Year Forward Plan and the broader Health & Social Care Strategy.
The Strategic Health Liason Officer facilitates the strategic engagement of the voluntary and community sector (VCS) with statutory bodies in Stoke on Trent and Staffordshire (particularly Northern Staffordshire) as well as promoting mutual understanding between the two sectors so that they can work better together in partnership and collaboration.
This engagement between the VCS and statutory bodies enables the VCS to participate in the co-design and redesign of health and social care services for the local population and keeps the local VCS informed of developments within the health and social care arena, both locally and nationally that may impact on commissioning and have relevance to their service users.
VCS Hub – Social Prescribing
The Health Service teams provides a referral service to individuals and professionals involved in their health and social care, to promote the local VCS as an option for improving the health and wellbeing of patients and residents. This increases choice, promotes resilience and reconnects individuals to their communities, increasing their social connections and networks which can lead to improved health and wellbeing.
